Well Visits:

ALL CHILDREN UNDER 18 MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN FOR WELL VISITS. 

Prior to well visits, we strongly encourage parents to download the forms from our website and complete them in advance, or arrive 10 minutes early to complete forms. 

 

On Time Policy:

Please be on time for all appointments! Unfortunately, if you and your child are more than 15 minutes late for a well visit, we will have to reschedule the appointment.  We understand that things do happen, so if you know you are going to be late, please call!

No Shows:

If your child is unable to keep an appointment, we require 24 hour notice so his/her time can be made available for another patient.  Unfortunately, we will have to assess a fee of $25.00 for each appointment that is not cancelled in a timely manner. 

 

Sick/Well together:

There are times when the provider is doing a well check that a sick visit needs to be done at the same time. This is done as a courtesy to you so that you do not have to return to the office on a different day. Both services will be billed to your insurance company and you will be responsible for the usual balance after your insurance pays.

 

After hours calls:

Open Skype, search for CMGOSBY, and send a message stating the child's name, date of birth and the problem.  The doctor will call you through your Skype app.  If you don't hear anything in 15-20 minutes, please send a video call to CMGOSBY.  If you don't get an answer in 15 minutes, please call 410-546-3173 and push 1 to be connected to our answering service.

These video visits will be billed to your insurance company. You will be responsible for the usual copay/balance after your insurance pays.

 

Referrals:

We require 3 days notice for completion of referral forms.  When calling, please have ready the name and contact information for the specialist, as well as the date/time of appointment.

 

School/Camp Forms:

Please allow 3-5 business days for completion.

 

Prescription Refills:

Please allow 3 days notice for medication refills.
